Admission / Referral Process
Admission Criteria
Maple Manor treats patients with spinal cord injury, traumatic brain injury, joint replacements and fractures, cardiac and respiratory failure, wound care, neurological conditions, other medically complex conditions. Admissions criteria includes:
- Diagnosis
- Medical stability at time of transfer
- Rehabilitation goals
- Identification of insurance for prior authorization
- Disposition at discharge
Maple Manor’s programs are designed for adult patients, 18 years of age of older.
Maple Manor accepts patients on ventilators and has a high success rate of ventilator weaning and management. Maple Manor’s ventilator unit has limited space and admission decision is made on a case-by-case basis by the Maple Manor physicians and Admission Director.

Referral Process
Admission referrals to Maple Manor come directly from the hospital. The hospital case manager will discuss a referral and fax medical records and insurance information either electronically via ECIN/Allscripts or by fax to 248-624-8810.
Patients, families, physicians, and external case managers are encouraged to ask the hospital case manager to make the referral.
Once the referral is received, Maple Manor’s admissions team will contact the insurance carrier to verify benefits.
For approved admissions, Maple Manor’s admissions team will coordinate with the hospital discharge team and discuss the date of transfer to Maple Manor.
